This floor mat must be installed correctly to prevent it from moving into the accelerator. Do not use if damaged or the metal loops are missing." It may even include a drawing of a foot with a crumpled floor mat up against a pedal. Soon, every new car sold in America comes with a similar warning.
Well, the original floor mats that came with my Tundra *did* come with a large tag attached to them saying that you shouldn't stack floor mats and to ensure that the original mat is hooked on properly to avoid pedal jamming.

I did end up experiencing a similar issue with sticking pedals because I replaced the OEM carpet-mats with generic rubber ones, but didn't cut them. So out came the tin snips to take a chunk off the corner, and ziptied the whole thing to the seat anchoring brackets to prevent sliding around.
